I wanna subscribe to dolly?!?

I want to subscribe to dolly but i dont have one of there mag's and i dont want to do it over the internet. (with pay pal or ccredit card) I want to pay in cash and send it threw good old fashioned mail. does anyone know where you can find a good print-able form version that i can send in mail. anyone know where to find one? and plz dont say the dolly website because that doesnt work. thanks. x
